A recent VA probe found Fort Collins VA staff under McCormack's leadership as well as the staff of the Cheyenne VA were taught to falsify appointment records going back at least one year.
The goal of falsifying the records was to make the small staff of doctors appear they were seeing patients within the 14 day timeline requirement.
There was also a "bad boys list" in Cheyenne for anyone who did not submit false reports.
McCormack is also under fire for receiving $8200.00 in bonuses despite these documented problems.
She says she met her performance standards and got her bonus based on "information available at the time."
